Community Outreach & Patient Engagement Specialist
Job Summary
The Community Outreach & Patient Engagement Specialist will serve as a trusted liaison between our hospital and the community. This role combines healthcare knowledge with community-based marketing and patient navigation. This role requires an active presence in the community, including outreach at outreach events, neighborhood engagement, and relationship-building in socially or economically challenging areas.
The ideal candidate will have experience as a Medical Assistant or healthcare support professional, with strong interpersonal skills and the ability to engage diverse populations respectfully and be culturally sensitive.
Essential Functions
- Represents the hospital at community events, churches, health fairs, housing communities, and local organizations.
- Conducts outreach activities both indoors and outdoors.
- Engages residents in meaningful conversations about available healthcare services.
- Assists individuals in understanding how to access care and schedule services.
- A collaborative that builds trust within underserved communities.
- Identifies barriers to care (transportation, language, insurance concerns) and communicates them to leadership.
- Supports patient recruitment and retention initiatives.
- Maintain detailed outreach documentation and tracking reports.
- Collaborate with clinical and administrative teams to support growth and community impact.
- Assists with other community outreach responsibilities as needed.

Minimum Education/Certifications
- High school diploma or equivalent required.
- Medical Assistant certification or prior experience in a healthcare setting preferred.
- Valid driver’s license preferred.
Minimum Work Experience
- 1 year of community outreach experience in a behavioral health or acute care setting preferred.
- Basic knowledge of medical terminology and patient care processes.
- Strong communication and engagement skills.
- Comfortable working in diverse and sometimes challenging environments
- Professional, confident, and culturally sensitive
- Self-motivated and able to work independently
- Ability to attend community events, including occasional evenings and weekends
Position Type/Expected Hours of Work:
- Full-time (37.5 hours weekly)
- On-site position with 80% spent at assigned community locations and events.
- Day shift with occasional weekend or holiday coverage as needed.
Physical Requirements:
The position requires prolonged periods of standing. Positions will require stooping, bending, lifting, and carrying items weighing up to 25 pounds. Position requires the ability to work at outdoor events.
